This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Dental Services:   Prophylaxis Cleanings  Periodontal scaling and root planning  Simple and some Surgical Extractions  Restorative and other preventative work such as:  Oral Exams  Cleanings  Fillings New Horizon Family Health Services serves all patients regardless of ability to pay. However, services are not provided free of charge. New Horizon Family Health Services is a Federally Qualified Health Center (FQHC) and receives some federal funding to help offset the cost of providing care to patients on a sliding fee scale. Eligibility for the sliding fee discount is determined by documented annual income and family/household size. Patients with a documented annual income of 200% or below the federal poverty level (FPL) are eligible for a sliding fee discount on all services they provide, including prescriptions written by New Horizon Family Health Services providers and filled at New Horizon Family Health Services pharmacies. Hours:  Monday , Wednesday and Friday, 8:00 am - 5:00 pm.  Tuesday and Thursday, 8:00 am - 7:00 pm.
