This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Full dental services for all ages are provided at the Davis County Clinics and Midtown Dental Clinic at Midtown CHC of Ogden. Dental Services Include:  Extractions  Fillings  Root canals  Partials  Crowns  Dentures They accept all private dental insurance plans and offer reduced rates for those without insurance. Patients without medical insurance are welcome and will be charged at the time of service on a discounted sliding fee scale based on income and family size. Dental Hours:  Monday – Friday, 7:30 am - 6:00 pm.
