This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Communicare Health Centers provides comprehensive dental care for children and adults. To qualify for the discount program, you must have a total family income that is less than 200% of the Federal Poverty Income Guidelines. To assist us in assessing your eligibility for the program, you will need to provide the following required documents at your initial member registration: proof of identification, income, and residency as described below.
