The sliding fee scale applies to patients without health insurance & also do not qualify for Medicaid. The sliding fee scale is based on household size & the poverty guidelines set by the U.S. Department of Health & Human Services & its Board of Directors. Myrtle Hilliard Davis provides high quality & affordable health services. Patients are required to meet the income eligibility by submitting one or a combination of the following for proof of household income:     A couple of your most recent check stubs for all members of the household    Letter from employer stating annual income    W-2 forms for all family members for previous year    Federal Tax return from previous year    Social security/entitlement statement from previous month    Letter from Division of Employment SecurityInstructions:    List all charges to the patient at 100% level.    Verify family size, read down left hand column to proper family size, read across that row until salary range is found, then read up to find the percentage.    Multiply the percentage found times the total charges to arrive at balance due from patient.    The patient charges will be a percentage of the total or $20.00 for medical & $40.00 for dental whichever is greater    A minimum charge of $20.00 of medical & $40.00 for dental will be assessed to all patients including 0% patients    If a patient presents no conclusive proof to establish their sliding fee scale eligibility, place the patient on 100% pay until proof is established.
