This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Dental ClinicHours: Mondays, Tuesdays 7am to 5pm. Wednesdays, Thursdays 8am to 5pm. Fridays 7am to 4pm. If you do not have insurance, we have a SLIDING FEE available to qualified individuals!  This plan can significantly reduce the cost of your office visits.  Just call our office to inquire!Payment is expected at the time of service for all cash paying patients.  If you have insurance and are expected to pay a co-pay and/or deductible, that amount is due on the day of service.Insurances we participate with are Medicare, Medicaid Michigan, BCBS of Michigan – Traditional, PPO, PPO Trust, MiChild, Medicare Plus Blue & Advantage.  BlueCare network HMO-PCP assigned to subscriber must be one of our doctors.  Meridian Medicaid, McLaren Medicaid & Molina.Commercial Insurances we participate with are Aetna/Cofinity, Priority Health – Caid/Commercial/Care, Tricare North 2 & 5, WPS Tricare, , McLaren Health Advantage – Caid/Care/ Commercial Plans, Allied Benefit Systems, First Health Network, all Humana Plans, Coventry and Assurant.We will courtesy bill for all commercial insurances.
