Provide a current year tax return for you and your spouse. You must earn no more than 200% of the 2015 federal income guidelines: 1-person household $23,540; 2-person $31,860; 3-person $40,180; 4-person $48,500; 5-person $56,820; 6-person $65,140; 7-person $73,460; 8-person $81,780. For families with more than 8 persons, add $4,160 for each additional person.Eligibility ScreeningPatients must be screened for eligibility before seeing a physician. Screenings are offered every Thursday from 2–4pm. No appointment is necessary. Please bring the following items to your screening: Picture ID. Social Security card. Letter from your employer stating that you are employed and you do not have health insurance. If your spouse is working, a letter from your spouse’s employer stating that there is no insurance that covers you. If you are self employed, bring an up-to-date Profit and Loss statement and Schedule C. The last six payroll slips for you and your spouse. Two utility bills with your name and address to confirm Bibb County residency. Current year’s 1040 tax return for you and your spouse. If you file separately from your spouse, we consider both incomes.
